Credentialing Requirements
Education & Certification

Health Coaching Certification:
Nationally recognized health coaching certification (NBHWC, CHES, AAHC, or equivalent). Evidence of continuing education required. -
Clinical Background (for RN/MD coaches):
Active clinical license (RN, RD, LPC, MD, DO, etc.) in good standing with no disciplinary actions. License verification through state boards required. -
Foundational Education:
Minimum bachelor's degree or equivalent professional credentials. Transcripts verified and documented. -
Continued Competency:
Minimum 20 continuing education hours annually in health coaching, clinical practice, or related field with documented evidence.

Comprehensive seven-year criminal history check. Any felonies disqualify. Misdemeanors reviewed for relevance to coaching role. -
Sex Offender Registry Check:
National and state sex offender registry screening. Any registration results in disqualification. -

State child abuse and elder abuse registry screening. Any substantiated findings disqualify applicant. -
Professional License Verification:
Direct verification with state licensing boards. Confirmation of active status, no suspensions, and no pending disciplinary actions.

Session Documentation Requirements
All coaching sessions must be documented according to the following standards:
- Timeliness: Documented within 24 hours of session completion
- Client Demographics: Name, DOB, MRN, contact information, insurance verified
- Session Details: Date, time, duration, mode (in-person, phone, video), location/setting
- Chief Complaint/Focus: Primary health concern or coaching focus for session
- Assessment: Client status, motivation level, barriers identified, readiness for change
- Coaching Interventions: Specific evidence-based techniques used (motivational interviewing, goal-setting, etc.)
- Goals & Action Plans: SMART goals established, specific action steps, timeline, accountability measures
- Outcome Measures: Clinical outcomes tracked (A1C, BP, weight, etc.), behavioral outcomes, engagement metrics
Quality & Compliance Standards
Documentation must meet the following quality and compliance standards:
- Clarity & Legibility: Clear, organized, free of jargon, understandable to healthcare team
- Accuracy: Factually correct, objective language, no opinions or judgments
- Completeness: All required fields completed, no blank spaces without explanation
- Signature & Authentication: Electronically signed by coach with date/time stamp within 24 hours
- HIPAA Compliance: No PHI in external documents, secure access controls, audit trails maintained
- Billing Support: Documentation supports medical necessity and CPT code selection
- Legal Defensibility: Professional standard of care evident, outcomes documented, compliance clear
- Retention: Maintained minimum 7 years in secure repository with backup

Renewal Expectations & Timeline
Two-Year Credential Cycle
NHCN credentials are valid for two years, with renewal requirements designed to ensure ongoing competency and professional development.

Renewal Requirements Checklist
To renew credentials, coaches must meet all of the following requirements:

Renewal Approved
Coach receives notification of renewal approval, updated credential certificate, and extended credential status through expiration date
Renewal Pending
Coach remains in practice with active credentials while renewal documents are under review. Final decision expected within 30 days
Renewal Denied
If renewal requirements not met, coach receives formal denial, explanation, and appeal rights. Credentials lapse on expiration date
NHCN Governance & Oversight Structure
Credentialing Committee
The NHCN Credentialing Committee oversees all credentialing decisions and ensures consistent application of standards.
