1. Entry Into the Network
Patients enter NHCN through four primary channels: direct‑to‑patient, clinic‑driven, coach‑driven, and health plan & employer referrals. Regardless of how they arrive, every patient begins with the same unified, guided experience.
2. Intake & Personal Assessment
Patients complete a structured intake that captures health goals, lifestyle habits, medical context, preferences, availability, and insurance information. The intake is warm, supportive, and clinically meaningful, giving coaches and clinics the context they need to help.
3. Personalized Recommendations
Based on the intake, NHCN generates a personalized coaching plan, a summary of goals, and a clear explanation of how coaching can help. Insurance‑aware options ensure patients understand their coverage and care pathways.
4. Intelligent Matching
The NHCN matching engine evaluates goal alignment, coach specialty, availability, insurance compatibility, clinic affiliation, and patient preferences. Patients are matched to a coach first or a clinic first depending on their needs and plan requirements.
5. Coaching Begins
Once matched, patients can schedule their first session, message their coach, and begin guided onboarding. Coaching sessions are structured, supportive, and outcomes‑driven, with documentation aligned to clinical and payer expectations.
6. Ongoing Support & Progress Tracking
Patients receive session summaries, progress dashboards, habit tracking, educational resources, and ongoing encouragement. Coaches document each session using standardized templates that support quality, consistency, and clinical integration.
7. Clinic Integration (When Needed)
If a coach identifies clinical needs — or if the patient entered through a health plan or employer program — NHCN seamlessly connects the patient to an appropriate clinic. Clinics receive structured summaries and clear next steps, creating a closed‑loop care model.
